FSGS stands for Focal Segmental Glomerulosclerosis. It describes scarring in the kidney. These are what the words mean:

Focal - only some of kidneys' tiny filters called glomeruli have been damaged, while some are still normal;

Segmental - damage has occurred to only a section of an individual glomerulus;

Glomerulosclerosis - the scarring of the glomeruli.

Causes

What causes this kidney disease is often unknown, doctors tend to put the disorder into two big categories, primary and secondary.

- Primary means that the disease happens on its own, for no obvious reasons.

- Secondary means that the disease is caused by, or it associated with, another medical condition. Some of these secondary causes/associations may include kidney defects from birth, urine backing up into kidneys, obesity, obstructive sleep apnea, sickle cell anemia, viruses (like HIV), etc.

Having family members with FSGS does increase one's chance of develop the disorder. The disease is twice as common in blacks than in whites.

Tests & Diagnosis

▪ Conventionally, a blood test, urine tests, and a kidney biopsy will determine if a person has glomerulosclerosis.

▪ Immune special tests

- Kidney damage tests. Checking items include: UTP, U-malb, U-TRF, U-IGG, β 2-microglobulin, α1-microglobulin, α2-macroglobulin, κ light chain, λ light chain, U- NAG, U-GGT, and Uosm.

- Urinary protein electrophoresis.

- Selective urine protein index (SPI).

- Complement in blood serum test. This test include C3, C4, CH50, C3B, etc.

- Toxins in blood test. Checking items include: BUN, Creatinine, UA, β 2-microglobulin, Cyc C, RBP, HCY, and PTH.

What are the chances of getting better?

Left untreated, most people with this disorder will eventually progress to complete renal failure wherein dialysis or a kidney transplant is needed to sustain life. Over half of patients develop into Chronic Kidney Failure within 10 years.

Kidney Transplant in FSGS

FSGS symptoms

People with early stages of FSGS may not experience any symptoms. With progressive damage of kidney function, patients will present the following signs and symptoms.

Swelling

Swelling, or edema, especially in the legs are the most common detection among patients. Often they find their shoes no longer fit, or that they have suddenly put on weight.

Protein in urine

Normally, urine contains only a very small quantity of protein, as it is too big to pass through the kidney's tiny filters (glomeruli). However, if these filters become scarred or damaged, they can "leak" protein into the urine. People with a large amount of protein in their urine may find their urine looks foamy or frothy.

High Blood Pressure

High blood pressure, or hypertension, is also a very common symptom. The elevated blood pressure can often be very difficult to treat.

Tiredness or fatigue

Healthy kidneys can secrete a hormone known as erythropoietin (EPO) that tells the body to make oxygen-carrying red blood cells. With failed kidneys, there is not enough production of EPO and red blood cells. In this case, patient would feel tired very quickly.

Difficulty breathing

This problem may occur due to two reasons: excessive accumulation of fluid in the lungs and anemia (shortage of oxygen-carrying red blood cells).

Nausea and/or vomiting
