Nephrotic Syndrome (NS) refers to a condition that is often caused by any of a group of diseases that impair the kidneys' tiny filters, the glomeruli. The urination of over 3.5 g/d is the primary indicator of this kidney disorder. About 2 in every 10,000 individuals are affected by this disease. In children, it is diagnosed in more boys than girls, usually between 2-3 years of age.

Causes

A number of different disorders can cause this kidney disease. Diabetes and to a lesser extent, high blood pressure may cause diffuse impairment to the glomeruli and can eventually result in NS.

The following conditions may cause specific damage to the glomeruli and often result in the development of heavy protein in the urine and in many instances NS:

Amyloidosis, congenital nephrosis, FSGS, Glomerulonephritis, IgA Nephropathy (Berger's disease), minimal change disease, pre-eclampsia, etc.

Tests & Diagnosis

In addition to a medical history and physical exam, other tests to diagnose Nephrotic Syndrome include:

▪ A 24-hour urine collection, which measures the total amount of protein in urine collected over 24 hours.

▪ Blood test. Decreased serum albumin level may lead to fluid retention in the ankles, abdomen, or lungs.

▪ Creatinine and creatinine clearance tests results give information on how well your kidneys are functioning.

▪ Kidney ultrasound to look at the kidneys. This exam can rule out other causes of your discomforts and symptoms.

In adults, testing usually includes:

▪ Serum protein electrophoresis (SPEP)

▪ Antinuclear antibodies (ANA)

▪ Antibody tests for SLE

▪ C3 and C4 complement, proteins normally found in the blood

▪ Tests for hepatitis B, hepatitis C, and HIV

Unless NS occurs clearly due to Diabetes, a kidney biopsy is often done to confirm the exact cause. Kids don't often have a biopsy.

Prognosis

The prognosis varies. The disorder may be acute and short-term or chronic and not respond to treatment. Complications that occur can also affect the outlook.

Untreated or not treated effectively, some people may ultimately progress to Kidney Failure wherein dialysis or a kidney transplant is necessary.
